    HealthInsight 2006-11 Quality Award

    Haven HealthCare received the HealthInsight Quality Award for six consecutive years from 2006 to 2011. HealthInsight is the Medicare Quality Improvement Organization for Nevada and Utah that looks at performance on publicly reported quality of care measures from clinical and administrative data collected from home health agencies nationwide. The award is given to agencies that meet the criteria of ranking in the 90th percentile of patient outcomes and have no condition of participation level deficiencies in the Utah state survey.

    2008-09 HomeCare Elite™

    Haven HealthCare was named to the 2008 and 2009 HomeCare Elite™, a compilation of the top-performing home health agencies in the United States. Awardees are selected for being among the top 25% of the providers in quality outcomes, quality improvement, and financial performance measures. The performance recognition is based on data compiled from publically available quality measures, and is brought to the industry by OCS HomeCare, a leading provider of homecare information, and DecisionHealth, publisher of the independent newsletter Home Health Line.

Haven HealthCare is a full-service home health care agency, offering both medical and non-medical home care to families throughout the Salt Lake Utah area. Our service area includes Salt Lake City as well as Ogden, Provo and surrounding cities. Services include nursing care, physical and occupational therapy, post-hospital rehabilitation, extended care, wound care, home health aides, medical social workers, medication management, and hospice care. Home caregivers and personal care assistants provide help with activities of daily living, senior and eldercare, Alzheimer’s and dementia care, transportation and companionship. We are an affiliate of Kindred at Home.
