1. After printing the certificate, call 801-676-6000.
2. Tell the receptionist you are purchasing a Gift of Care certificate.
3. Tell us the amount of the gift. We accept VISA or MasterCard.
4. You will be given a Gift ID Number to hand write onto the certificate.

When your loved one calls to activate the order, he or she will give us the code, and all necessary arrangements or billing adjustments will be implemented. Your thoughtful gift is certain to be appreciated.
