Mission Statement

Haven Healthcare's hospice program provides an individualized program of physical, emotional, spiritual, and practical care for people in the last phases of a life-limiting illness with an emphasis on control of pain and other symptoms. The program is reflective of a spirit and idea of caring that emphasizes comfort and dignity for the dying, making it possible for them to remain independent for as long as possible, and in familiar surroundings. Hospice care is centered on both the patient and family/caregiver. Hospice personnel respect and respond to the unique differences in family, lifestyle, culture, values, and wishes.

Our Care

We employ an interdisciplinary team approach to create a plan of care according to a patient's individual needs and desires. Our dedicated team of medical directors, nurses, hospice aides, social workers, chaplains, trained volunteers, and bereavement specialists provide well coordinated, loving care. In addition, families and caregivers receive emotional support and assistance to care for their loved one while making sure that end of life comes naturally and peacefully. What is Hospice?


Hospice emphasizes
palliative rather than curative treatment.

Hospice affirms life and regards dying as a normal process.

Hospice neither hastens nor postpones death.

Hospice provides personalized services and a caring community so that patients and families can attain the necessary preparation for a death that is satisfactory to them.

Insurance

We accept Medicare and other selected private insurances. Medicare pays for hospice care if you or your loved one meets the following criteria:

1. A patient elects hospice care and waives any rights to other curative benefits related to the terminal illness. However, the patient has the right to choose to discontinue hospice care and return to medical care at any time.

2. A patient's physician and our Medical Director certify that a patient's life expectancy is under six months. 

Volunteer

Volunteers are part of our hospice team and provide supportive emotional, spiritual, and other services to patients and families who are facing difficulties at the end of life. Visiting a patient’s home, a volunteer serves as a friend to offer respite for caregivers and emotional support to patients and their loved ones. Activities could include reading to the patient, playing board games, assisting with a hobby, or simply listening. Volunteers may also help by running errands or shopping for groceries. Volunteers become part of Haven's Interdisciplinary Team, where they participate in case conference, in-services, and hospice training.
